Registered Nurse Nurse Led Outreach Team

The Registered Nurse (RN) is a regulated health care professional who is accountable and responsible for providing quality patient/resident centered nursing care to individuals living in long term care (LTC). As part of the inter-professional health care team the RN will practice according to the nursing practice expectations set by Osler the regulatory practice standards of the College of Nurses and the legislative requirements set by the Regulated Health Professions Act and the Fixing Long Term Care Act (FLTCA).

Oslers Nurse Led Outreach Team (NLOT) is a mobile outreach team which visits 23 long term care (LTC) homes in the central west catchment as well as all Osler sites. The NLOT RN will implement the best practice patient/resident-centered care delivery based on the NLOT model. NLOT is a nurse practitioner outreach model of care that involves supporting LTC residents care needs through on site bedside visits in LTC home or hospital bolstering capacity of Registered Practical Nurses in LTC and facilitating sustainable discharges.

In this role you will use high levels of emotional intelligence passion for the care of residents in LTC and the ability to relate to others to ensure adoption of best practice and improve transitions.

Accountabilities:

• You will work collaboratively within the NLOT model to implement evaluate and sustain the foundational mandate of the team
• You will support the NLOT mandate and model by attending education related to the model of care
• You will inspire and lead others to effectively implement quality care in LTC (LTC national standard and FLTCA) through role modelling coaching and through communication
• You will participate in change methodology to support and sustain quality care standards in LTC and support integration as it relates to the NLOT model of care
• You will incorporate your knowledge of transitions and resident-centered care strategies to support those living in long term care to age in place
• Safe nursing practice in all aspects of the nursing process: assessment planning implementation and evaluation of patient/resident care
• Compliance with policies and procedures associated with observations
• Professional patient-centered care to ensure patient safety and physiological and psychological well-being associated with creating a therapeutic relationship and environment
• Providing care in a compassionate and caring manner within a diverse cultural environment
• Practicing according to the infection control and prevention measures
• Providing health teaching to residents families and care providers including thorough discharge instructions to ensure the continuity of care and a sustainable discharge

Qualifications :

• Current registration and in good standing with the College of Nurses of Ontario required
• BCLS required
• Experience within the last five years with a minimum of two years in acute medicine required (bedside nursing skills)
• Experience working with geriatric dementia and palliative patients within the last two years required
• Demonstrated excellent interpersonal communication problem-solving conflict-resolution and decision-making skills required
• Geriatric certificate or enrollment preferred
• Gentle persuasion approach (PIECES is preferred)
• Demonstrated experience in care of patient with tracheostomy suprapubic and foley catheters wound care enteral feeds IV therapy and isolation
• Knowledge and understanding of information systems and technology required
• May be required to travel and work between LTC homes and all Osler sites (access to a vehicle is required)
• Vulnerable sector check or attestation as it pertains to the FLTCA required

• Demonstrated knowledge of patient/resident and family-centered care
• Demonstrated experience with patient and family health teaching preferred

• Demonstrated excellent customer service organizational and critical-thinking skills required
• Demonstrated commitment to continuous learning and professional development as it applies to the profession and related field
• Demonstrated ability to work in a stressful and fast-paced environment
• Exemplary clinical practice within an inter-professional team to develop appropriate strategies for patient/resident care preferred
• Ability to prioritize and make autonomous decisions required
• Excellent physical assessment skills required
• Ability to promote a healthy work environment and practice excellent client safety guidelines
• Knowledge and understanding of the Occupational Health and Safety Act

Hours: Currently days including the potential for weekends evenings and statutory holidays based on program needs (subject to change in accordance with operational requirements)
