Job Summary
Eden HHC is looking for Certified Home Health Aide (CHHA) to join our dedicated service area includes Los Angeles County Orange County Ventura County San Bernardino County Riverside County. We use Wellsky/Kinnser for EMR. Position is for Per will set preferred areas to cover.

DUTIES & RESPONSIBILITIES
- Helps patient with bath care of mouth skin hair and/or other personal hygiene activities.
- Helps patient in and out of bed.
- Assists patient with ambulation use if wheelchair and/or walker as directed.
- Helps with prescribed exercises.
- May prepare meals and help patients with eating (per prescribed diet directed).
- Reinforces therapeutic diet instructions.
- Performs household services which facilitates the patients self-care at home and are necessary to prevent or postpone institutionalization.
- Helps patient to bathroom or in use of bedpan/urinal.
- Reports changes in patients condition and/or needs to supervise nurse and appropriate therapist.
- Works independently in home with only intermittent RN supervision.
- Schedules own home visits (per visit frequency established by agency).
- Assists patient in taking medications that are normally self-administered.
- Charts record of patient care on progress notes after each visit.
- Completes other appropriate records.
- Provides for a safe and sanitary environment for the patient adheres to Agencys Infection Control and Safety Management policies.
- Participates in and/or conducts team conferences.
- Participates in other specific duties as assigned and defined by Agencys policy.
- Performs all activities incorporating and applying the principles of proper body mechanics.
- Participates in orientation staff education team meetings and patient care.
- Implements care based on Plan of Care and Agencys policies.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT/EQUIPMENT USED:
Sympathetic attitude toward the care of the sick. Ability to read write and carry out directions. Maturity and ability to deal effectively with the demands of the job.
Work is of medium demand walking standing and driving. Lifting moving and turning of patients. Pushing wheelchairs and/or other equipment.
Hearing for physical assessment skills near visual acuity color vision.
Nursing supplies and equipment.
EDUCATION EXPERIENCE TRAINING LICENSES:
Verifiable aide care in the home hospital or related employment.
Successful completion of a Home Health Aide training program as recognized by the State of California for certification.
Current Home Health Aide Certificate issued by the State of California.
Current CPR certification.
Current California Drivers License. Automobiles Automobile Insurance.
Current Vaccination status
