Home Care Marketer / Care Manager

Location: Monmouth and Ocean County NJ
Type: Full-Time

The Role
This is a hybrid position combining community marketing and relationship development with care management and client support. Youll be the face of our agency in the community and a trusted guide for families once they come on board.

Youll help us grow thoughtfully and strategically while ensuring the families we serve feel supported informed and genuinely cared for.

What Youll Do

Community Growth & Marketing

Build and nurture relationships with hospitals rehab centers assisted living communities physicians elder law attorneys and financial planners
Represent the agency at networking events senior expos and community programs
Identify creative outreach strategies (lunch & learns educational workshops digital engagement ideas)
Track referral sources and develop growth strategies
Collaborate with leadership on branding messaging and positioning

Care Management & Client Support

Conduct initial client assessments (home visits when needed)
Serve as a primary point of contact for new families during onboarding
Help families understand care plans schedules and available resources
Coordinate with caregivers and internal staff to ensure quality service delivery
Problem-solve proactively when challenges arise
Maintain accurate documentation and communication

Qualifications

Experience in home care healthcare marketing case management or senior services
Strong communication and presentation skills
Valid drivers license and reliable transportation
Familiarity with the local healthcare and senior services landscape is a plus
Bachelors degree preferred but not required
