Job Description:
Licensed Practical Nurse
$3000 Sign-On Bonus Available

On an Average Day You Will:
(includes but not limited to)
Provide skilled nursing care under the direction of an RN or physician
Follow and verify the Plan of Care at each visit
Communicate changes in condition to the RN Case Manager and physician
Document care medications and patient progress accurately
Educate patients and families on self care and safety
Help prevent avoidable hospitalizations and ER visits
What You Bring:
Graduate of an accredited nursing program
Active Indiana LPN license
At least one year of clinical experience
Home health or hospice experience preferred
Valid drivers license insurance and reliable transportation
Ability to lift push pull or support up to 50 pounds
