About the Role
A well-established home health and hospice organization is seeking a compassionate Physical Therapist to join its care team across multiple territories in Pennsylvania and Michigan. This is a full-time community-based role for PTs who enjoy working independently building relationships with patients in their homes and being part of a collaborative clinical team that genuinely cares about outcomes.
Key Responsibilities
- Deliver skilled physical therapy services to patients in residential home settings
- Develop and implement individualized care plans in coordination with the broader care team
- Complete clinical documentation accurately and on time using EMR software
- Manage your own schedule and caseload within the assigned territory
- Travel locally to patient homes approximately 20% of your day is driving
Qualifications
- Bachelors or Masters degree in Physical Therapy from an accredited program
- Active PT license in the state where youll be working (PA or MI)
- Minimum 1 year of healthcare experience (new grads may be considered)
- Valid drivers license and reliable transportation
- Experience with EMR documentation preferred (HomeCare HomeBase a plus)
- Ability to self-manage time schedule and meet charting deadlines
