Job Description:
- Provides crisis intervention/rapid response and suicide risk assessment services as well as individual and group psychoeducation related to better management of health/behavioral health issues and to risk reduction of recidivism to criminal justice system under the supervision of an independently licensed professional.
- Develops and coordinates community re-entry plans that address risk factors associated with recidivism to the criminal justice system.
- Maintains knowledge of community resources and contacts to support patients effective transition back to the community.
- Completes suicide risk and bio-psychosocial assessments community re-entry plans and other clinical reports/mental health forms in timely fashion and under the supervision of an independently licensed professional.
- Obtains treatment records and coordinate care with family community providers and others relevant to healthcare and community reentry plan.
- Completes and coordinates requests for mental health related Court Ordered Evaluation (COE) with community providers and testifies in court as needed.
Requirements
Minimum Qualifications:
- Diploma/transcripts to support graduation with a bachelors degree in behavioral health social services or a related field.
- Minimum one year of past work experience in this position.
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
