Job Description:
- Coordinates regarding psychosocial treatment issues related to patient care.
- Presents relevant findings that pertain to mental status and patient risk in an acute environment.
- Prepares comprehensive bio-psychosocial assessments mental status exams treatment plans and other clinical reports.
- Provides individual treatment and crisis intervention services. Facilitates therapeutic and psychoeducational group treatment.
- Conducts short term solution-focused therapy with the use of treatment plans goals and objectives.
- Develops treatment plans for patients with serious mental illness and co-occurring disorders.
- Maintains familiarity with mental health diagnoses that correspond to International Classification of Diseases (ICD) and Diagnostic and Statistical Manual (DSM).
- Coordinates treatment with Maricopa County Sherriffs Office (MCSO) courts and community providers for continuity of care.
Requirements
Minimum Qualifications:
- Diploma/transcripts to support graduation with a masters degree in behavioral health social services or a related field.
- Current valid unrestricted license as a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) issued by the Arizona State Board of Behavioral Health Examiners.
- Current completed PSV from the Arizona State Board of Behavioral Health Examiners.
- Minimum two years of past work experience in this position.
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
