Job Summary

Job Description:

  • Obtains history and completes physical examinations of patients incarcerated in the County jails.
  • Evaluates and treats patients as medically indicated in compliance with CHS policies and procedures.
  • Documents patient encounters according to CHS protocols in a Subjective/Objective/Assessment/Plan/Education (SOAPE) format in the progress notes or on the appropriate CHS clinical form.
  • Develops and updates individualized treatment plans for patients with chronic conditions or special needs.
  • Orders onsite lab work radiology studies treatments and medications as medically indicated.
  • Refers patients to external service providers including specialists diagnostic studies and other services as medically indicated.
  • Performs minor procedures such as sutures incisions and drainage of abscesses simple splints punch biopsies and pelvic exams.
  • Collaborates and communicates with hospitals medical providers community agencies state and federal agencies and other counties regarding continuity of medical care.
  • Participates in peer review in-service training quality improvement processes and other meetings necessary to improve patient care.


Requirements

Minimum Qualifications:

  • Diploma/transcripts to support graduation from an accredited NP program.
  • Current valid unrestricted license as an Adult NP or Family NP issued by the Arizona State Board of Nursing.
  • Current completed PSV from the National Council of State Boards of Nursing; including verification of "Prescription Authority".
  • Current certification in Advanced Nursing Practice from the American Nurses Credentialing Center (ANCC) or copy of current certification as an Adult NP or Family NP from the American Academy of Nurse Practitioners (AANP).
  • Current DEA/Controlled Substance Registration Certificate to prescribe Schedule II III and IV drugs.
  • Current completed NPI verification.
  • Current completed fit testing for N95 particulate respirators.
  • Minimum two years of past work experience in this position.



Required Skills:

Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff


Required Education:

Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
