Job Description:
Maintains familiarity with various medications their proper dose use and contraindications. Accurately prepares and fills medication containers with prescribed medication in the quantity ordered and affixes labels. Maintains records of all pharmacy transactions. Performs inventory control duties. Maintains orders labels and stocks pharmaceuticals and supplies. Accepts and reconciles deliveries and ensures proper storage of medication. Assists with medication transfers between clinics. Communicates with nursing staff daily to verify any new medications changes in therapy or discontinuations. Prepares and organizes medications for pharmacist to perform quality assurance by accurately reading verifying and following medication orders and prescriptions.
Requirements
Minimum Qualifications:
- Copy of current valid unrestricted certification as a CPhT issued by the Arizona State Board of Pharmacy.
- Copies of current completed PSVs obtained via the Arizona State Board of Pharmacy and Pharmacy Technician Certification Board.
- Minimum one year of past work experience in this position
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
