Job Description:
- Provides routine dental treatment for patients diagnosed with having dental disease.
- Provides extractions fills and denture repairs.
- Performs dental examinations on patients diagnoses dental conditions and develops treatment plans.
- Records all treatment rendered to the patients in the dental record.
- Provides instruction to the patient about oral hygiene and post-surgical care.
- Triage patients request for dental care and prioritizes according to CHS policies and procedures.
Requirements
Minimum Qualifications:
- Diploma/transcripts to support graduation with a Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) from an accredited program.
- Current valid unrestricted license as a Dentist issued by the Arizona State Board of Dental Examiners.
- Current completed PSV from the Arizona State Board of Dental Examiners.
- Current Drug Enforcement Agency (DEA)/Controlled Substance Registration Certificate to prescribe Schedule II III and IV drugs.
- Current completed NPI verification.
- Minimum two years of past work experience in this position
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
