UW Medical Center-Northwests Long Term Civil Commitment (LTCC) Department has an outstanding opportunity for a Peer Advocate Wellness and Recovery at the Center for Behavioral Health and Learning Building.
WORK SCHEDULE

Support the individual to advocate for themselves and act as a champion of hope and recovery
DEPARTMENT DESCRIPTION
Wellness Peer Advocates provide mentorship and develop trusting relationships with patients to communicate hope and encouragement along the care continuum from contact on admission through to community transition.
PRIMARY JOB RESPONSIBILITIES
Assist patients during their inpatient LTCC hospitalization and the 90 days post discharge to overcome obstacles and identify their own health wellness and recovery goals
Provide one on one support during inpatient LTCC hospitalization and the 90 days following discharge focusing on building partnerships and developing trusting relationships
Partner with patients to assist with the identification and linking to community-based services and supports
Work as a part of the patients treatment team completing inpatient and outpatient groups documenting in the patients medical record
Partner with treatment team to facilitate a successful discharge and transition into the community
Partner with Occupational Therapy and treatment team to assist patients with transitional independent living skills accompanying patients into the community to establish supports
REQUIREMENTS
High school graduation or equivalent i.e. GED
One year of professional experience as a Peer Advocate/Peer Counselor
Peer Counselor Certification (Department of Behavioral Health and Recovery) OR Community Health Worker Certification (Department of Health) in WA State.
PREFERRED
One year of experience partnering with a hospital organization in a Peer Liaison capacity
College degree in health and human services field or equivalent.
