About The Position
As a CarePoint Intake & Support Specialist you serve as an initial point of contact for community members seeking support through this role you gather key information learn about individuals needs and help them understand what services may be available. The position blends strong interpersonal communication with organized intake and referral practices ensuring each person receives a welcoming informative and efficient introduction to OCOs programs and resources. Using OCOs Universal Service Delivery approach the Intake & Support Specialist builds trust with consumers provides early support and helps them navigate available addition to direct client interaction this role plays an essential part in keeping the CarePoint program running smoothly by assisting with documentation data entry recordkeeping communication with partners and other day to day administrative tasks. This position is a great fit for someone who is organized empathetic and passionate about helping people access meaningful support.
JOB SUMMARY:
Provides direct services according to OCOs Universal Service Delivery methods in diverse locations including OCO offices partner agency sites and community settings. This position serves as the primary intake worker for the CarePoint Program conducting screenings eligibility assessments and initial documentation for individuals with potential Health-Related Social Needs (HRSNs). The Specialist maintains a small caseload of low-acuity consumers requiring monthly follow-up and provides program support functions to ensure smooth daily operations. Duties are carried out in accordance with Agency and Program policies and procedures. Carries out duties in accordance with Agency and Program policies and procedures.
JOB DUTIES AND RESPONSIBILITIES:
- Provides crisis intervention supportive counseling advocacy and solution focused decision making for individuals and families.
- Works in collaboration with other service providers to engage consumers and assist them in connecting to needed services.
- Informs consumers of available services in the community and assists with the referral of primary and secondary consumers to services and monitors the provision of services to all consumers.
- Conduct screenings eligibility assessments and collect documentation required to help determine eligibility for agency programs.
- Provides or arranges transportation to enable consumers to connect with needed community resources.
- Helps maintain caseload and waitlist logs for CarePoint program.
- Communicates pertinent information to co-workers supervisors and community partners as instructed.
- Complete initial data entry in required systems such as OCOs Universal Database and program required systems.
- Completes all documentation as required for documenting and invoicing for billable services as required.
- Provides case support for consumers who require minimal monthly follow-ups including invoicing case notes and creating/updating service plans; identifies when additional needs emerge and escalates cases to care management staff as appropriate.
- Order track and organize program and office supplies.
- Help maintain organized files accurate documentation and compliance with agency expectations.
- Support program activities reporting needs and quality improvement efforts.
- Attend internal and external meetings as assigned.
- Perform other duties as assigned.
JOB REQUIREMENTS:
- Desire to work with individuals and families; understanding of family development and OCOs Service Delivery model.
- Must be able to work independently and be self-motivated resourceful organized and adaptable in carrying out job duties
- Must be familiar with the geographical layout of the county and have knowledge of human service resources in the county
- Must have good oral and written communication skills and have excellent documentation and reporting skills.
- Must possess skills for complex data entry and have the ability to read and interpret complex information and manipulate data.
- Must have demonstrated ability to work with others in a warm non-judgmental manner and be a positive role model.
- Must be able to direct the work of others set priorities and problem solve.
- Must exhibit professionalism good judgment and maintain strict confidentiality with regard to client information.
- Must exhibit a detailed knowledge of regulations services rights and responsibilities for the population served.
- Must have the ability to work with populations with special needs including those that may exhibit challenging behaviors.
- Flexibility with scheduling to meet program needs.
- Must have acceptable health to carry out the responsibilities of the position.
- Must have a valid NYS Drivers license with driving record within policy and access to a properly insured reliable vehicle for regular travel and transporting consumers.
MINIMUM QUALIFICATIONS:
- Must have an Associates Degree in Human Services or related field; and
- Must have 1 to 3 years experience with children youth families or a special population; or
- Any equivalent combination of education experience and training.
