Overview

We are seeking a full-time in-person Family Medicine Physician to join a mission-driven multidisciplinary outpatient healthcare organization serving medically underserved communities. The organization operates multiple outpatient clinics and provides care to a diverse patient population.

Physicians collaborate with an integrated care team that includes primary care physicians specialists advanced practice providers behavioral health clinicians and dental professionals. Key Responsibilities

Provide comprehensive primary care to adult and pediatric patients in an outpatient setting
Diagnose treat and manage acute and chronic medical conditions
Deliver preventive care health maintenance and chronic disease management
Collaborate with interdisciplinary clinical teams to coordinate patient care
Maintain accurate and timely documentation in the electronic health record (EHR) system
Participate in quality improvement initiatives and population health programs
Educate patients and families on treatment plans preventive care and wellness strategies

Qualifications

MD or DO with completion of an accredited Family Medicine residency program
Board Eligible or Board Certified in Family Medicine
Active state medical license (or ability to obtain)
Commitment to providing high-quality care to underserved populations
Strong clinical judgment communication skills and team-oriented mindset
