About the Role Corus Home Health in Albuquerque is seeking a compassionate Physical Therapist to join our dedicated home health team. Key Responsibilities Evaluate patients and create personalized therapy plans in the home setting Deliver evidence-based interventions to restore mobility and function Educate patients and families on exercises and safety strategies Collaborate with the interdisciplinary team to ensure coordinated goal-oriented care Maintain accurate and timely documentation to meet quality and compliance standards Adapt care plans based on progress and physician feedback Support patients through discharge planning and wellness strategies Requirements What We re Looking For Licensed Physical Therapist (PT) in the state of New Mexico At least 1 year of clinical experience (home health preferred) Skilled in patient communication documentation and problem-solving Self-motivated and comfortable working independently Proficient with electronic health records and mobile charting Passionate about function-focused relationship-driven care Committed to our mission: to bring out the courage in others Why Join Us Corus Home Health is more than a provider we re a therapist-owned and operated organization built on empathy empowerment and clinical integrity.
