Pay Rate:
$32.50 $35.0/hr

Position Overview
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join the care team at Autumn Lake Healthcare at Old Bridge. The LPN will provide direct patient care assist with treatments administer medications and support the healthcare team in delivering high-quality care to residents.
Key Responsibilities
Administer medications and treatments as prescribed by physicians
Monitor and record patient vital signs and health status
Assist in developing and implementing patient care plans
Provide direct patient care and support daily living activities
Document patient information accurately in the medical record system
Communicate effectively with physicians nurses and healthcare staff
Ensure compliance with infection control and safety procedures
Educate patients and families on care plans and treatment instructions
Requirements
Active LPN license in New Jersey
BLS certification preferred
Experience in long-term care or skilled nursing facility preferred
Strong communication and patient care skills
Ability to work in a fast-paced healthcare environment
