Job Summary
Were looking for a Nurse Practitioner who loves primary care and values building real relationships with patients. Our busy well-established primary care practice in the Farmington Valley is growing and were excited to add a compassionate team-oriented APRN to our group. We pride ourselves on providing thoughtful patient-centered care in a supportive and collaborative environment.
This is an outpatient position with a strong support team and an established patient base. Youll have the opportunity to care for a diverse adult population manage both acute and chronic conditions and truly make a difference in your patients lives.

Responsibilities:
- Provide comprehensive primary care to adult patients
- Perform physical exams diagnose and treat acute and chronic conditions
- Order and interpret diagnostic tests
- Develop and manage treatment plans
- Educate patients on preventive care and wellness
- Maintain accurate and timely documentation in the EMR

- Connecticut-licensed APRN (FNP or ANP)
- Primary care experience preferred but strong new graduates are welcome
