Job Summary
Job Summary (List Format):
- Evaluate residents functional abilities including ADLs mobility cognition and psychosocial factors.
- Develop and implement individualized occupational therapy treatment plans based on evaluations and physician orders.
- Provide direct occupational therapy interventions to improve residents functional independence and quality of life.
- Monitor document and adjust resident treatment plans to achieve rehabilitation goals.
- Participate in interdisciplinary care planning meetings and collaborate with other healthcare team members.
- Educate residents families and caregivers on safety and strategies to promote independence.
- Recommend adaptive equipment and assistive devices as needed to support resident function.
- Supervise and provide guidance to Certified Occupational Therapy Assistants (COTAs) and therapy aides.
- Ensure accurate timely and compliant documentation in electronic medical records.
- Maintain professional licensure and complete required continuing education.
- Support quality improvement initiatives and comply with CMS and facility regulations.
- Work within a skilled nursing facility (SNF)/long-term care environment requiring teamwork physical activity and strong communication skills.
