- Assess diagnose and treat residents with speech language cognitive-linguistic and swallowing disorders in a skilled nursing facility.
- Develop and implement individualized treatment plans to promote resident independence safety and quality of life.
- Conduct comprehensive evaluations and provide direct therapy services for communication cognition and swallowing.
- Perform and interpret swallow assessments; make recommendations for diet modifications as needed.
- Document all evaluations treatments progress notes and discharge summaries according to regulations.
- Educate residents families and staff on therapy plans communication strategies and safe swallowing practices.
- Participate in care planning and interdisciplinary team meetings contributing to resident-centered care goals.
- Monitor resident progress and adjust treatment plans as appropriate to achieve optimal outcomes.
- Ensure compliance with professional legal and regulatory guidelines.
- Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs) as required.
