Job Summary: We are seeking a dedicated and compassionate Occupational Therapist (OT) for a 13-week contract assignment in a Skilled Nursing Facility setting. The ideal candidate will provide high-quality rehabilitative care to patients helping them regain independence and improve daily functional abilities.
Key Responsibilities: -
Evaluate and assess patients physical cognitive and emotional abilities

Develop and implement individualized treatment plans

Provide therapeutic interventions to improve activities of daily living (ADLs)

Collaborate with interdisciplinary team members to coordinate patient care
-
Educate patients and families on treatment plans and home exercise programs
-
Maintain accurate and timely documentation of services provided
Qualifications: -
Active and current state Occupational Therapist license

Experience in a Skilled Nursing Facility preferred

Strong communication and interpersonal skills

Ability to work independently and as part of a team

BLS certification (if required by facility)
