Job Summary
Job Title: Licensed Practical Nurse (LPN) Long Term Care

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a 13-week contract assignment in a Long Term Care setting. The LPN will provide direct patient care to residents ensuring safety comfort and quality clinical outcomes in accordance with facility standards and nursing best practices.
Key Responsibilities:-
Administer medications and monitor for therapeutic effectiveness

Perform patient assessments and document findings accurately in EPIC EMR

Provide skin treatments and wound care including wound vac management
-
Assist residents with Activities of Daily Living (ADLs)
-
Administer nebulized treatments
-
Provide CPAP care and gastrostomy tube care
-
Collaborate with interdisciplinary team members to ensure comprehensive patient care

Participate in weekend holiday and overtime rotations as required
Varied 8-hour rotating shifts:
-
Days: 7:30 AM 3:30 PM

Evenings: 3:00 PM 11:30 PM
-
Nights: 11:00 PM 7:30 AM
Includes weekends and holidays. Overtime rotation may be required. Hours and shift preference are not guaranteed.
Requirements:-
Active Hawaii LPN license required by the start of assignment

Minimum of 1 year of Long Term Care experience

Current BLS/BCLS certification
-
Proof of influenza vaccination or approved medical/religious exemption required prior to start

Ability to arrive prior to start date for required employee health clearance appointment
