Job Title: Licensed Vocational Nurse (LVN) Night Shift

Schedule: Night shift with 4-3 / 4-3 rotating block schedule

Job Summary The Licensed Vocational Nurse (LVN) provides direct nursing care to residents under the supervision of a Registered Nurse. This role is responsible for supporting continuity of care through medication administration resident monitoring and coordination with the interdisciplinary team. The LVN will also supervise CNAs and support staff and may be required to cross-train across nursing departments as needed.
Key Responsibilities -
Assess plan and implement resident care under RN supervision

Provide direct patient care and administer medications safely and accurately

Monitor resident condition and report changes promptly

Supervise CNAs and support staff to ensure quality care delivery

Maintain accurate and timely documentation using Electronic Medical Records
-
Collaborate with the care team to support continuity and quality outcomes

Participate in cross-training across nursing departments as required
Qualifications & Requirements -
Active California LVN license required

Minimum of 2 years Skilled Nursing (SNF) experience required (Sub-Acute experience does not qualify)

Current BLS certification required

Ability to pass required nursing competency/medical testing

Experience with Electronic Medical Records preferred (PointClickCare or similar)

Strong verbal and written English communication skills

Professional interpersonal skills when working with residents families staff and external partners

Basic computer proficiency required
