Position Overview

The Home Health Manager plays a critical role in overseeing skilled home healthcare services for seniors ensuring participants receive high-quality care in their homes nursing homes and board and care facilities. This position involves managing the home health team coordinating care across disciplines and supporting the delivery of individualized care plans.

Key Responsibilities

Direct a high-performing home care department ensuring workflows policies and standards are consistently followed.
Guide and manage home care staff including hiring training and coaching team members.
Assess the effectiveness of home health nursing assessments and interdisciplinary team (IDT) participation.
Ensure home care nurses deliver and document interventions according to participants care plans.
Oversee the quality of care provided in nursing homes and board and care facilities.

Required Qualifications

Registered Nurse (RN) license in California.
Minimum of 2 years of nursing experience in a home health setting preferably with frail or elderly populations.
Strong clinical knowledge to manage complex scenarios and provide care for elderly participants.
Bachelors Degree preferred (additional experience may substitute).
