Your Opportunity
An exciting opportunity is available for a recovery-oriented AOD Case Manager to join Partners Toward Wellbeing (PTW) in Keswick SA on a permanent full-time basis. PTW is a voluntary community-based service supporting adults living with alcohol and other drug use and/or mental health challenges helping them work toward their own wellbeing goals. You will provide short- to medium-term relationship-based support including counselling care coordination and case management with a focus on choice dignity harm reduction and recovery. Support is delivered in the community and tailored to what matters most to each person.
As a Case Manager you will work with people and their families offering practical emotional and relational support helping them navigate challenges build on strengths and connect with supports they choose. You will also contribute to group programs including skills-based groups (e.g. DBT-informed skills) life skills harm minimisation education peer-led recovery groups and initiatives shaped by lived experience.
This service is funded by Adelaide PHN through the Australian Governments PHN Program and is free to participants. Lived experience is valued and recognised as a powerful source of connection hope and recovery. Your key responsibilities will be to:
Deliver person-centred recovery-oriented case management for adults experiencing alcohol and other drug use and co-occurring mental health challenges.
Collaborate with participants to develop implement and review individual well-being and case plans supporting progress toward goals that are meaningful to them.
Provide counselling group work brief therapeutic and strengths-based interventions advocacy and supported referrals to internal and external services.
Support participants to navigate systems services and everyday challenges fostering confidence self-direction and connection.
Maintain accurate timely and respectful documentation including intake forms case notes goal plans reports and data entry in client management systems.
Build and sustain collaborative relationships with families carers and service providers to support coordinated holistic care.
Contribute to continuous quality improvement reflective practice and team learning while maintaining professional boundaries and Mission Australias values.
Requirements for Success
Certificate IV or higher in Alcohol and Other Drugs Case Management Social Work Community Services or a related field or equivalent relevant experience.
Lived experience of alcohol and other drug use and/or mental health recovery is highly valued and encouraged.
Experience or interest in supporting individuals and their families experiencing AOD-related challenges.
Ability to build positive collaborative relationships with clients families carers and service providers.
Excellent communication organisational and record-keeping skills with the ability to manage a complex caseload and competing priorities.
Hold a current South Australian Drivers Licence.
Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
