permanent Job Title: Registered Nurse / Licensed Clinical Social Worker - Case Management Company: Progeny HR Solutions LLC Location: Atlantis FL USA Contract Details: Full-time permanent Progeny HR Solutions LLC is seeking a highly skilled and compassionate Registered Nurse / Licensed Clinical Social Worker to join our team as a Case this role you will be responsible for providing comprehensive case management services to patients in the Atlantis FL area. As a Registered Nurse / Licensed Clinical Social Worker you will work closely with patients and their families to assess their needs develop care plans and coordinate services to ensure the best possible outcomes. You will also collaborate with healthcare providers and community resources to advocate for the physical emotional and social well-being of our patients. Responsibilities: - Conduct comprehensive assessments of patients physical emotional and social needs - Develop individualized care plans based on assessment findings and in collaboration with patients and their families - Coordinate and facilitate services to meet patients needs including medical care mental health services and community resources - Monitor and evaluate the effectiveness of care plans and make adjustments as needed - Educate patients and their families on disease management treatment options and available resources - Advocate for patients rights and needs within the healthcare system - Maintain accurate and up-to-date documentation of patient interactions and services provided - Collaborate with interdisciplinary teams and participate in case conferences to ensure coordinated and comprehensive care - Stay current on industry trends best practices and regulations related to case management and healthcare Requirements: - Current and valid Registered Nurse (RN) and Licensed Clinical Social Worker (LCSW) licenses in the state of Florida - Minimum of 2 years of experience in case management preferably in a healthcare setting - Strong assessment care planning and coordination skills - Excellent communication and interpersonal skills - Ability to work independently and as part of a team - Proficient in electronic medical records and documentation - Knowledge of community resources and healthcare services in the Atlantis FL area - Compassionate patient-centered approach to care - Ability to prioritize and manage multiple tasks efficiently - Flexibility to adapt to changing needs and priorities - Bilingual (English/Spanish) skills a plus We offer competitive salary and benefits packages as well as opportunities for professional growth and development.
