Responsibilities:
  • AWAREs SDMI Referral Coordinator is a person who has the responsibility of coordinating and facilitating referrals for the SDMI Waiver.
  • Review and upload documents regarding clinical and financial eligibility for SDMI Waiver to the members chart.
  • Manage SDMI Waiver Waitlist to include documenting referral information receiving communications regarding waitlist information and maintaining quarterly contact with waitlist members.
  • Upload annual Level of Impairment forms provided by Mountain Pacific to member record.
  • Follows State of Montana and A.W.A.R.E. Inc. rules policies procedures and guidelines as they apply to this service.
  • Must demonstrateon a consistent basisthe willingness and ability to productively utilize work time in order to meet the needs of the individuals being served as directed with the understanding that specific fiscal goals must be met monthly.
  • Record referrals and other data entry assignments via AWAREs Electronic Healthcare Record System.
  • Engage in follow up assessment of need and referrals to SDMI Waiver services.
  • Coordinate with Clinical Administrator to support SDMI Waiver enrollment process.
  • Engage in routine communication with SDMI Waiver providers regarding service availability.

Requirements


Talents skills and abilities:

  • AWAREs Adult Case Manager must have the interpersonal oral and written communications skills to work as a team member and to follow oral and written instructions.
  • A AS/AA Degree and minimum of one (1) year of direct work experience is preferred. An education equivalency can be established with combination of education and pertinent employment experience.
  • Must have or be willing to obtain a valid Montana Drivers License; along with maintaining an acceptable driving record; as well as an acceptable criminal and protective services background check.
