Southcity Clinic is a busy community-based specialist drug and alcohol treatment service specialising in pharmacotherapy treatment approaches for people with complex comorbidities. Clinical services are based around a harm-minimisation philosophy of care and include assessment medical treatment counselling and psycho-social care coordination.
What youll be doing
The AOD Liaison Outreach Worker (ADLOW) is a position designed to support improved access to existing AOD services and will be based within Southcity Clinic. The role will work closely with the Alfred Consultation-Liaison Addictions team to link Alfred consumers with high intensity AOD support needs into relevant community services.
The role will work with complex AOD clients whose needs will be best serviced through the provision of specialist AOD supports to achieve the nest outcome for the client through a single and seamless transition with clearly focussed goals and outcomes. Clients will be referred to ADLOWs when their primary health provider determines that they require more intensive specialised support.
