Responsibilities
The Patient Navigator Liaison is responsible for assisting in various aspects of care management to ensure that the highest quality of care is being rendered resources utilized are appropriate and services provided are in the appropriate setting. Works collaboratively with other departments and physicians to facilitate timely efficient and appropriate management of all care across the continuum.

Qualifications
High School Diploma High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
Completion of a course of study consistent with the requirements for Medical Assistants as specified by the Medical Board of California required
5 years experience in Care Coordination/Case Management activities required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
