Job Summary:

The Permanency and Youth Services Unit supports transition age youth who are experiencing foster care. The Unit strives to offer youth centered supports and services in order to ensure young people are prepared to live outside of the foster care system.
The Transitional Housing Coordinator will support housing opportunities for young adults in Extended Foster Care. They will work with community partners contracted providers DCS Specialists and youth to determine which housing option best meets their needs. They will provide technical assistance to Contracted Provider as well as oversight and monitoring of the programs to ensure a positive experience for young adults.

Job Duties:
  • Accept and assist youth in completing Self Assessment Application with youth. Take youth on site visits of housing options. Coordinate with placement administration for youth who have to return to licensed placements. Complete needed documentation in Guardian including Service Authorizations.
  • Track housing census weekly. Providing general information about housing screening of applicants for acceptance into various housing programs as well as verifying eligibility for such opportunities.
  • Facilitating referrals and serves as advocate on behalf of youth to ensure provider is assessing appropriate the in services and supports are in place for youth to reach their goals. Coordinate between DCS and Contracted Providers.
  • Provide monitoring training site visitation.
  • Participate in staffings with providers assist in the resolution of conflict. Determine if youth needs alternative housing option work with the DCS Specialist and Provider on attainment of new living arrangement.
  • Other duties as assigned as related to the position.

Knowledge of:

  • Social services federal state and local programs and resources regarding area of assignment
  • Terminology of Arizonas child welfare system processes including Youth Thrive Framework
  • DCS policies and procedures; child welfare policies; general policies pertaining to investigating reports of abuse/neglect
  • Trauma informed practice
  • Group facilitation conflict resolution
  • DCS related programs; program planning; group dynamics; and evaluation process

Skills in:

  • Oral written and interpersonal communication
  • Interviewing and eliciting information;
  • Oral organization and management of workload
  • Problem identification and solutions

  • Facilitation and coordination of culturally diverse meetings

  • Represent and make appropriate decisions for DCS when working as a liaison in regard to interactions with DCS Staff other agency personnel and community partners and providers

Selective Preference(s):

Experience working with adolescents or young adults.

Pre-Employment Requirements:

Bachelors degree from an accredited college or university in education social work psychology counseling or other related fields with 1 year of experience in working directly with young people in Housing programs or Arizonas foster care system. (Masters degree from an accredited college or university in one of the above mentioned fields will substitute for one year of experience.) or an Associates degree in an accredited college or university in education social work psychology counseling or other related fields with 3 year of experience in working directly with young people in Housing programs or Arizonas foster care system.

Requires the possession of and ability to retain a current valid state-issued drivers license appropriate to the assignment. Employees who drive on state business are subject to drivers license record checks must maintain acceptable driving records and must complete any driver training (see Arizona Administrative Code R.11). Employees may be required to use their own transportation as well as maintain valid motor vehicle insurance and current Arizona vehicle registration. Mileage may be reimbursed if applicable. (Arizona Administrative Code Revised 1/29/2020)

Must be able to secure and maintain an Arizona Fingerprint Clearance Card.

Must be able to secure and maintain clearance from DCS Central Registry.
