Job Summary
Job Title: RN Case Manager
Location: Syracuse NY
Duration: 13 Weeks (Contract)
Shift: Day Shift 5x8 Hours (07:00 AM 03:00 PM)
Compensation
Weekly Gross Pay: $2614
Position Summary
A healthcare organization is seeking an experienced RN Case Manager for a 13-week contract assignment. This role supports care coordination utilization review and discharge planning in an acute care setting ensuring high-quality compliant and patient-centered outcomes.
Key Responsibilities
Conduct utilization review to ensure medical necessity and regulatory compliance
Collaborate with physicians and clinical teams to clarify and improve clinical documentation
Develop implement and monitor individualized care and discharge plans
Educate patients families and staff on care plans and documentation best practices
Monitor patient progress and identify and resolve barriers to care
Maintain accurate and timely documentation within the electronic medical record
Support quality initiatives coding accuracy and compliance with regulatory requirements
Required Qualifications
Experience
Minimum of 1.5 years of recent acute care experience (within the past 2 years) in a comparable facility
Experience with EPIC Electronic Medical Records preferred
Licensure & Certifications
Active New York State RN License
Basic Life Support (BLS) American Heart Association
