Harborview Medical Centers Pediatric Clinichas an outstanding opportunity for aCommunity Health Worker Early Childhood Developmental Navigator.

WORK SCHEDULE

  • 100% FTE
  • Days
  • This position is expected to be a 2 year duration.

POSITION HIGHLIGHTS

  • Partner with families to support early childhood development including developmental promotion screening and connection to community-based resources
  • Integral part of the primary care health team

PRIMARY JOB RESPONSIBILITIES

  • Conduct person-centered screenings for development behavior and social stressors for children ages 0 to 65 months
  • Share screening outcomes with the medical provider and maintain open communication with the care team
  • Identify and connect families to early childhood community-based resources including but not limited to Early Services for Infants and Toddlers Child Find Home visiting Early learning/Preschool
  • Develop and maintain trusted relationships with community partners and school districts
  • Maintain timely documentation of assessment and intervention activities in the electronic health record (EHR). Track needed follow and collect demographic data per grant/funder requirements
  • Participate in professional development regarding anti-racism oppression and cultural competency and regularly assess own performance and program for related improvements

REQUIREMENTS

  • High school graduation or equivalent and two years experience in a behavioral health organization or consumer/peer run organization; or equivalent education/experience.
  • AND Peer Counselor Certification (Department of Behavioral Health and Recovery) OR Community Health Worker Certification (Department of Health) with WA State within 6 months of employment.
  • Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements such as a license certification and/or registration

DESIRED QUALIFICATIONS

  • Strong Bilingual English/Spanish communication skills
  • Outstanding organizational skills and ability to manage a caseload of multiple families
  • Interest and ability to connect with diverse families and support them in overcoming systemic barriers
  • Comfort with interacting with young children and commitment to creating a child-friendly environment
  • Ability to collaborate effectively with diverse patients and healthcare team members
  • Commitment to improving the experience of care for children and families particularly those underserved by our current systems.
  • Sufficient technological capabilities to use electronic medical records and other systems to record visit notes collect data and track patient progress.
  • Ability and desire to be flexible and responsive
  • Access to transportation to attend occasional off-site visits trainings or meetings.
  • Ability to obtain and/or maintain current certifications including Pediatric/Adult CPR first aid and AED HIV and blood borne pathogens
  • Knowledge of early childhood development
  • Knowledge of local resources for families with young children preferably resources in King County
  • At least 1 year of work or life experience in early childhood (parenting/education/developmental services) home visiting social support services infant mental health or anything else related to early childhood
