Job Summary

The HFW Youth Support Partner (YSP) is a young adult who was personally involved in a child-serving system as a youth allowing them to have a unique understanding of the youth they serve. The YSP assists assigned youth in identifying prioritizing and working towards goals. The YSP attends monthly interagency HFW Planning Meetings with assigned youth encouraging them to speak up about their own strengths culture and needs. The YSP will ensure youth and their team members understand one another and come to agreements whenever possible. As the youth works towards their goals the YSP helps them to address any barriers that arise and keeps the team updated on progress.

  • Offer suggestions and develop solutions to help promote effective and efficient work processes and innovative programs.
  • Actively participate in required meetings and complete all mandatory trainings.
  • Maintain high levels of advocacy and member confidentiality to ensure the success of CBH and our mission.
  • Assist in assuring both internal and external program integrity by being alert to and reporting suspected instances of provider and employee fraud waste or abuse.

Essential Functions:

  • Maintain regular contact and attend monthly planning meetings for a caseload of up to 25 youth
  • Attend all mandatory county and state trainings
  • Adhere to the 10 Principles of High-Fidelity Wraparound
  • Follow the 4 phases of High-Fidelity Wraparound and all activities associated with each phase
  • Ensure that the Principles and steps of the High-Fidelity Wraparound process are delivered with the highest possible fidelity
  • Assist youth in identifying and utilizing their natural supports
  • Identify and link youth to community resources
  • Ensure youth understand and agree with their individualized Wraparound Plans and Discovery documents
  • Monitor completion of youths action steps and assist them in addressing barriers
  • Attend at least 3 hours of group or individual coaching/supervision per week
  • Other non-essential duties as specified by the immediate supervisor

Position Requirements:

  • Education: High School Diploma or GED
  • License/Certification:
    • Valid PA Drivers License
    • HFW Youth Support Partner credential within 12 months of hire
    • Valid Clearances including Criminal Background Child Abuse and FBI must be received prior to start date
  • Relevant Experience:
    • Individual lived (personal) experience being involved in a child-serving system (DHS JJS mental health etc.) as a youth
    • Knowledge of various community resources youth/families in Philadelphia can access
    • A basic understanding of the child welfare and mental health systems in Philadelphia is preferred
  • Skills:
    • Ability to develop manage and maintain relationships with others
    • Ability to engage others through continued collaboration
    • Strong communication skills
    • Excellent documentation skills
    • Able to organize and manage time independently
    • Ability to identify strengths and culture of each youth and share with their team
    • Ability to work in the evening or on the weekend as needed to meet the needs of the youth
    • Can accommodate frequent travel within Philadelphia and surrounding suburbs and occasional out of state travel
    • Knowledge of community and natural supports which provide adolescents and families with opportunities to succeed

Minimum Qualifications:

  • Possess a High School Diploma or GED
  • Valid PA drivers license
  • Valid clearances including a criminal record child abuse and FBI
  • Access to a vehicle preferred but not required**

Work Conditions (including travel overtime required physical requirements and occupational exposure):

  • Must be able to travel frequently in Philadelphia and surrounding counties.
  • Out of state travel may be required occasionally.
  • Some evenings and weekends will be required to meet the needs of youth.
  • Regularly requires walking standing stooping bending sitting reaching pushing pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
  • Ability to utilize computers and office equipment to complete daily work responsibilities.
  • Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
  • Must be able to adapt to continuous changes/demands of the job.


Philadelphia Residency Requirement:

  • The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
