Full Spectrum Search Group is a full-service healthcare recruiting agency specializing in post-acute leadership and healthcare administration. We are seeking an experienced RN Case Manager Hospice in Cleveland Ohio for one of our hospice clients.
Position Details
- Position Title: RN Case Manager Hospice
- Location: Cleveland OH
- Experience: Active RN license required; hospice home health or home care experience preferred
- Compensation: $85000 annual salary plus additional compensation for on-call coverage

Qualifications
- Active RN license in Ohio
- Hospice experience preferred; home health or related post-acute experience considered
- Strong clinical assessment and care planning skills
- Ability to manage a defined caseload independently
- Strong communication skills with patients families and interdisciplinary teams
- Willingness to participate in an on-call rotation
Responsibilities
- Manage and coordinate hospice care for patients within a defined Cleveland territory
- Conduct comprehensive assessments and develop individualized care plans
- Collaborate with physicians and interdisciplinary team members to ensure quality care
- Educate patients and families regarding comfort-focused treatment and support
- Maintain accurate and compliant clinical documentation
- Participate in on-call rotation as scheduled
