Greenfield Rehabilitation Agency is seeking a dynamic and experienced Physical Therapist (PT) to serve as Rehab Director at our beautiful continuum-of-care senior community in Oconomowoc Wisconsin.

This is more than a leadership role its an opportunity to shape clinical excellence mentor a strong interdisciplinary team and make a daily impact on the lives of seniors across short-term rehab long-term care assisted living and independent living settings.

Rehab Director Responsibilities:

Lead and manage the Therapy Department to ensure high-quality efficient service delivery
Supervise mentor and develop therapy staff
Collaborate with interdisciplinary teams to support individualized resident care
Provide direct patient care while overseeing departmental operations
Manage staffing scheduling and productivity standards
Ensure compliance with state and federal regulations
Monitor clinical outcomes and implement continuous improvement strategies
Foster a positive accountable team-oriented culture
Maintain regulatory readiness and documentation accuracy
Advocate for residents to maximize functional independence and quality of life

Qualifications:

Active Wisconsin Physical Therapist (PT) license
Previous leadership or supervisory experience preferred
Strong operational organizational and team-building skills
Excellent communication and problem-solving abilities
Passion for senior rehabilitation and quality outcomes
Commitment to integrity compassion and clinical excellence

Qualifications

Graduate of an accredited school with a bachelors masters or doctoral degree in Physical Therapy
License as required by the State of Wisconsin to practice as a Physical Therapist
At least 1 year experience as a treating clinician required
