Job Summary:
The Registered Nurse (RN) with Wound Ostomy Continence (WOC) certification will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. The position may float between units within a rehabilitation facility.

- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Gather and report patient data.
- Perform therapeutic procedures.
- Document patient care and related duties.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- National Alliance of Wound Care and Ostomy Certification.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Two years minimum of hospital home health or hospice care experience.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- Position floats between units.
- Block schedules are required.
Scheduling:
- Weekdays only.
- 8 or 10-hour shifts may be considered.
- Weekends are not required.
- On-call is not required.
