    Description Of Work:

    • This is professional social services work. The Bilingual Early Intervention Service Coordinator (case manager) is responsible for service coordination and delivery of appropriate early intervention services for eligible infants and toddlers (birth to three) and their families as specified in P. L. 102-119. Individuals with Disabilities Education Act and Act 212 Early Intervention Services System Act of 1990.

    Other responsibilities include:

    • Service coordination is an active ongoing process that involves: assisting parents of eligible children in gaining access to early intervention services and other services identified in the individualized family service plan; coordinating the provision of early intervention services and other services (such as medical services) that the child needs or is being provided; facilitating the timely delivery of available services; and continuously seeking the appropriate services and resources necessary to benefit the development of each child being served for the duration of the childs eligibility. Transportation of child and family member to appointments as necessary.
    • Specific service coordination activities include; completing developmental screenings on children; coordinating and participating in evaluations and assessments; facilitating and participating in the development review and evaluation of individualized family service plans; assisting families in identifying available service providers; coordinating and monitoring the delivery of available services; coordinating with medical and health providers; and facilitating the developing of a transition plan to preschool services if appropriate.
    • The Bilingual Early Intervention Service Coordinator (case manager) obtains written parental consent in the parent(s) native language or mode of communication before conducting the initial screening process and the multi-disciplinary team evaluation; and initiating the provision of early intervention services.
    • The Bilingual Early Intervention Service Coordinator (case manager) shall advise the parents(s) of their rights as specified in 55 PA Code 4225 Procedural Safeguards for Children in Early Intervention Services.
    • If the review and interpretation result in a determination that the child is not eligible the Bilingual EI Service Coordinator (case manager) will notify the parent(s) in writing of this determination and their right to; review all data collected on the child and the family; and request a hearing on the recommendation.
    • If the review and interpretation result in a determination that an infant or toddler is ineligible for early intervention services but deemed at risk of developing substantial delays the Bilingual EI Service Coordinator (case manager) shall notify the parents(s) in writing of the disposition. The notice in the parent(s) native language or mode of communication shall inform the parent(s) that the program: has identified the child as eligible to be included in the screening and tracking component of the program which will include periodic re-screening or developmental checks performed by the service coordinator; the parent(s) right to review all data collected on the child and family; and the parent(s) right to request a hearing on the recommendation.
    • Maintain a case file for each individual on case list to include documentation of ongoing events to assure continuity of services.
    • Responsible for monthly completion of Early Intervention data into the mandated Pelican System utilizing laptop computer.
    • Prepare and complete service authorizations when warranted.
    • Prepare and complete data change reports for both state and internal record tracking systems.
    • Visit families in their homes children receiving services in facility-based programs and any other relevant community programs/hospitals to monitor the services being provided and to be aware of any changes that need to be facilitated.
    • Act as liaison for the family providers and supports to assure implementation of services.
    • Coordinate transition process between family their service providers/supports and the local school district.
    • Prepare and submit Service Documentation Forms on all contacts with family child provider and community in order to bill Medical Assistance.
    • Participate in supervisory conferences staff training sessions and meetings as scheduled. Maintain personal growth and development by attending seminars conferences and workshops specifically in child development.
    • Always maintain confidentiality and professionalism while performing the duties and responsibilities of this position.
    • Covering evening evaluations when needed.

    Work Schedule and Additional Information:

    • Full Time employment 37.50 hours per week.
    • Work hours are 8am-4pm Monday-Friday.

    REQUIRED EXPERIENCE TRAINING & ELIGIBILITY

    QUALIFICATIONS Minimum Experience and Training Requirements:

    • Six months of experience as a County Caseworker 1; or
    • Successful completion of the County Social Casework Intern program; or
    • A bachelors degree with a social welfare major; or
    • A bachelors degree which includes or is supplemented by 12 college credits in sociology social welfare psychology gerontology criminal justice or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
    • Any equivalent combination of experience and training which includes 12 college credits in sociology social welfare psychology gerontology criminal justice or other related social sciences.
    • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

    • This position falls under the provisions of the Child Protective Services Law.
      • Under the Law a conditional offer of employment will require submission and approval of satisfactory criminal history reports including but not limited to PA State Police clearance PA Child Abuse history clearance and FBI Fingerprint clearance.
