About Outpatient Clinics
Corewell Health operates over 300 outpatient clinics across Michigan bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.
Scope of work
Provides quality education counsel and support to clients and their significant others regarding diabetes self-care. Documents the education process patient information and clinical recommendations according to department/program/regulatory guidelines and measures outcomes to maximize continuity of care between settings. Participates in projects related to area of specialty and the development/revision of procedures/policies educational materials/guidelines educational programs. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community.
Qualifications
Required Bachelors Degree Nursing or related healthcare field
Preferred Masters Degree
3 years of recent relevant clinical experience including prior teaching experience with adult clientele Required
LIC-Registered Nurse (RN) - STATEMI State of Michigan required or CRT-Registered Dietitian (RD) - CDR Commission on Dietetic Registration required
CRT-CPR - AHA American Heart Association 90 Days required
CRT-Diabetes Educator (CDE) - NCBDE National Certification Board for Diabetes Educators 3 Yrs required
