Job Summary
Services are designed to help homeless veterans obtain housing employment mental health services and more.
What toexpect.
This is adirect careopportunity to provide support for mental health willassess andmonitorclient needsdevelopservice plans connect clients tocommunity services and resources andadvocate for yourclientsas necessary.
Additional responsibilities will include:
- Assistingclientsthroughprogram enrollment and orientation.
- Coordinatingclient medical appointments transportation housing money management etc.
- Developing andassistingwithclientmedication management plan.
- Obtaining andmaintainingentitlementsfor clients.
- Maintainingaccurateand complete documentation.
- Developing andcoordinatingtransitionsplans and program dischargesummaries.

Pay for this position is a flat rate of $20.34/hr.
What we need from you.
- One of the followingcombinationsof education and experience:
- Ahigh school diploma or GED and 2 years of related experience.
- A bachelors degreeand 1 year of related experience.
- The willingness to obtainadditionalcertifications andtrainingsasrequired.
- A drivers license with 3-points or less and access to an insuredvehicle.
- Must have availability toworkdays nights weekends and holidaysdepending uponneedof the program.
