Our client is looking for a Full-Time Home Health Physical Therapist to serve the College Station this role you arent just treating an injury; you are helping neighbors in the Brazos Valley reclaim their independence in the environment where they spend 90% of their time.
Key Responsibilities
- In-Home Evaluations: Conduct comprehensive initial assessments to determine functional limitations and safety risks within the patients actual living space.
- Functional Mobility Training: Design and lead therapeutic interventions focusing on transfers gait training and fall prevention tailored to the patients home layout.
- Case Management: Collaborate with a multidisciplinary pod including RNs Occupational Therapists and Physicians to coordinate the highest level of care.
- Home Safety Education: Teach patients and families how to utilize assistive devices and modify their environment to prevent re-hospitalization.
- Electronic Documentation: Maintain accurate real-time documentation (OASIS) using mobile-based EMR systems.
Qualifications
- Licensure: Active unencumbered Physical Therapist (PT) license in the state of Texas.
- Education: Graduate of an APTA-accredited Physical Therapy program (DPT preferred).
- Experience: 1 year of clinical PT experience required; previous home health experience is strongly preferred.
- Travel: Must have a valid drivers license reliable transportation and active auto insurance.
- Vibe: A self-starter who is comfortable working independently and can navigate the unique social dynamics of home-based care.
