THE ROLE:

The Community Development Manager drives territory growth by building and strengthening relationships with community-based referral sources and health plan partners. This role focuses on generating leads referrals and admissions through consistent in-field outreach strategic account development and value-based education. The Manager leverages structured sales planning performance data and community partnerships to expand access to non-medical caregiving services for underserved populations while achieving defined revenue and growth targets.

This is a hybrid position with up to 80% of time in the field.

WHAT YOU BRING TO THE TABLE:

Primary Responsibilities:

  • 12 years of sales business development or community outreach experience

  • Demonstrated success meeting or exceeding performance goals

  • Experience working in healthcare social services or managed care environments preferred

  • Valid drivers license and ability to travel extensively within assigned territory

  • Ability to work independently in a field-based role while maintaining structured reporting and accountability

Skills:

  • Strong relationship-building and networking capability

  • Effective presentation and public speaking skills

  • Strategic planning and territory management

  • CRM proficiency (Salesforce preferred)

  • Strong communication and value-based messaging skills

  • Organization and disciplined follow-through
