Schedule: 12-Hour Shifts 6:00 AM 6:30 PM
Hours: 36 Hours per Week (Full-Time)
Pay Rate: $44 per Hour
Position Overview
Navitas Healthcare LLC is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) for a full-time day shift opportunity in Phoenix AZ. This position focuses on delivering high-quality patient-centered care in a collaborative healthcare environment.
Key Responsibilities
Administer medications and treatments within scope of practice (oral enteral parenteral suppository etc.)
Perform sterile and aseptic procedures including wound care tracheostomy care gastrostomy/jejunostomy care and catheter care
Monitor and document patient responses to medications and treatments
Ensure adequate availability of medications and clinical supplies
Provide and receive detailed shift reports
Communicate effectively with residents families and interdisciplinary team members
Complete admission nursing assessments
Assist with Minimum Data Set (MDS) assessments quarterly reviews and care plan development
Maintain accurate and timely clinical documentation
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Attend and contribute to staff meetings
Qualifications
Knowledge of:
Geriatric and gerontological nursing principles
Effects of aging and age-related conditions
Team-based nursing practices and care plan implementation
Pharmacology and safe medication administration
Federal and state nursing regulations
Skills in:
Observing and reporting changes in patient condition
Interpreting nursing regulations and compliance standards
Assisting in development and execution of care plans
Effective oral and written communication
Building professional working relationships
Ability to:
Practice within nursing scope and regulatory standards
Assess and respond appropriately to medical emergencies
Make sound clinical judgments independently
Handle medical inquiries professionally
Work collaboratively in a team-based environment
