General Description

Advances the Agencys mission to promote health and well-being within the community by conducting in-person eligibility assessments for home-delivered and congregate nutrition services in accordance with applicable laws regulations and program standards.

Essential Functions:

  1. Ensures accurate eligibility and assessment for Home Delivered and Congregate Meals
  2. Collects and maintains accurate records and data
  3. Provides person-centered and culturally responsive services
  4. Protects consumers and reduces Agency risk
  5. Provide Nutrition Education to consumers as per the program standards

1. Ensures Accurate Eligibility and Assessment for Home Delivered or Congregate Meals

  • Conducts face-to-face assessments with participants to determine eligibility for home-delivered or congregate meal services in accordance with applicable federal and state requirements.
  • Applies program criteria and procedures to evaluate individual need and confirm qualification for services.
  • Completes annual reassessments or as required to determine continued eligibility.
  • Tracks participant reassessment schedules and maintains accurate documentation of assessment outcomes and actions taken.
  • Identifies participant risks or unmet needs and makes referrals to Aging and Disability Resource Connection (ADRC) Specialists or other community partners as appropriate.
  • Provides documentation and communication to support participant continuity of care and adherence to program standards.

2. Collects and Maintains Accurate Records and Data

  • Participates in outreach planning conducts community outreach and collaborates with program partners to support program goals.
  • Develops and implements strategies and activities to meet program goals in the areas of food nutrition nutrition education physical activity and self-management of chronic conditions.
  • Develops and maintains trusted relationships with program participants through professionalism compassion reliability and follow-through.
  • Motivates participants to engage in a variety of health promotion activities.

3. Provides Person Centered and Culturally Responsive Services

  • Embraces and exhibits the Agency Mission Vision and Core Values.
  • Provides excellent customer service meeting the needs of participants with courtesy professionalism and follow-through.
  • Interacts with others in a respectful culturally appropriate manner and utilizes language services as needed to communicate with individuals whose primary language is not English.
  • Maintains the skills and knowledge required to perform the job effectively seeking additional training and education as appropriate.
  • Collaborates with other Agency staff and community service partners to improve service access and participant outcomes.

4. Protects Consumers and Reduces Agency Risk

  • Follows all program and Agency policies procedures and regulations.
  • Maintains confidentiality and data security in accordance with HIPAA and Agency policy.
  • Accurately completes required documentation in accordance with federal state funding and Agency requirements.
  • Reports potential risks or service concerns promptly to the appropriate supervisor or designee.

5. Nutrition Education

  • Plan coordinate and deliver nutrition and health promotion education to Older Americans Act (OAA) nutrition program participants in accordance with federal and state requirements.
  • Ensure nutrition education is provided on a regular schedule quarterly education at congregate meal sites and at least annually for home-delivered meal participants.
  • Coordinate nutrition education activities under the oversight of the Program Manager and utilize approved educational resources from authorized agencies.
  • Maintain accurate documentation tracking and reporting of nutrition and health promotion activities to support compliance evaluation and audit requirements.

Experience and Skills:

Minimum Qualifications Education & Experience

A qualified applicant will have three (3) years of education and/or experience that demonstrates the knowledge skills and abilities required for the position.

Qualifying experience may include any combination of the following:

  • High school diploma or GED; and
  • Experience in social services nutrition services eligibility determination or community program coordination;
  • Experience working with older adults and/or individuals with disabilities in a social service setting;
  • Experience conducting client or participant assessments and maintaining accurate documentation.

Special Requirements

  • Background Check. All positions must pass a basic criminal history check. Conviction of a crime may not necessarily prohibit employment. Some may require additional clearance including but not limited to national or state fingerprinting.
  • Driving for Agency Business. Applicants must possess a current drivers license in their state of residence and acceptable driving history. May require proof of personal automobile liability insurance ORS Chapter 806 and use of a personal vehicle for business use.
  • Ability to travel throughout service area.

Required Knowledge Skills and Abilities

  • Knowledge of eligibility requirements for nutrition programs.
  • Ability to conduct in-person assessments and determine participant needs.
  • Knowledge of available social service programs and resources for older adults and people with disabilities.
  • Ability to collect analyze and report data accurately.
  • Experience with electronic data entry systems (such as GetCare).
  • Knowledge of confidentiality rules including HIPAA and mandatory reporting.
  • Ability to establish and maintain effective working relationships with program participants volunteers staff and community partners.
  • Skill in organization scheduling and time management to meet program timelines and documentation requirements.

Work Environment and Physical Requirements

Work is primarily performed in participants homes and office environments. The employee must be able to:

  • Travel frequently to participants homes and community locations within the assigned region.
  • Walk stand talk and listen with frequent interruptions throughout the day.
  • Grasp reach bend stoop and lift up to 30 pounds occasionally.
  • Sit push or pull as required for documentation and data entry tasks.
  • Drive an Agency or employee car and carry/use a laptop computer.
  • Contact with the public in home or office environments may risk exposure to irrational/hostile behavior contagious diseases or contact with domestic animals.

This job will typically be conducted during regular business hours but may require additional or irregular hours on occasion.
