Must know Kinnser or home care home base! We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at University Home Care Inc. As an LPN in home care you will provide high-quality nursing services to patients in their homes ensuring their comfort safety and well-being. This role requires excellent clinical skills strong communication and a genuine commitment to improving patients quality of life through personalized care.
Responsibilities
- Provide skilled nursing care to patients in their homes including administering medications monitoring vital signs and managing chronic conditions
- Collaborate with healthcare professionals and family members to develop and implement individualized care plans
- Document patient care accurately and timely maintaining all necessary records and reports
- Educate patients and families on health management medication usage and post-treatment procedures
- Ensure compliance with healthcare regulations and company policies
- Respond to patient emergencies and communicate changes in health status to supervising nurses or physicians
Requirements
- Valid LPN license in the state of Michigan
- Previous experience in home care or a related nursing field preferred
- Strong clinical assessment and patient care skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
- Reliable transportation to travel to patient homes within Livonia MI
