The Center is sex-positive and provides advocacy and critical health and social support services to support our community members through all stages of life focusing on our deeply held values of equity social justice and client-centered trauma-informed care. The Center currently has over 100 staff working at 7 facilities across San Diego County.

POSITION SUMMARY

Under the supervision and support of the Senior Director of Programs this position is designed to meet the individualized and critical needs of the transgender nonbinary agender demi-gender gender-fluid two-spirit genderqueer intersex gender nonconforming questioning and exploring community. The position will use peer-to-peer advocacy to assist client engagement with various organizations effectively navigating gaps and barriers faced by our gender-expansive community. This individual must have lived experience navigating various institutions and government agencies. They must work creatively and collaboratively with other departments/programs within The Center community and partners other LGBTQ guaranteeing culturally wraparound sensitive services to clients to improve the individuals overall safety stability health wellness and psychosocial needs. This team member must be self-determined enthusiastically client-centered and resourceful when advocating for the betterment and advancement of the gender- expansive community.

PRIMARY ROLES & RESPONSIBILITIES

Client Support:

  • Provide case management by creating individualized care plans to accomplish goals. Must be confident and remain grounded while navigating complex cases especially for those in crisis experiencing various types of trauma/abuse discrimination displays of self-harm housing insecurity and instability systemic/structural oppression and exclusion.
  • Provide appropriate linkages and referrals to gender-affirming and trans related care/treatment/support in collaboration with internal Center departments and external providers.
  • Navigate health insurance both state and federally funded ensuring access to equitable and trans-affirming healthcare. Informing and properly educating individuals about their rights they remain empowered during tedious processes as they can be traumatizing.
  • Assist the client in formally filing grievances and appeals with each person(s) insurance plans in addition to The Department of Managed Care Health.
  • Ability to follow up with patients medical referral(s) and prior authorizations for their medical. Prioritize consumers privacy and protected health information by remaining HIPAA compliant.
  • Assist clients in securing employment financial security and enrollment benefits to meet basic Aid folx experiencing housing insecurity socially displaced and/or in migration fleeing discrimination and violence from their country or place of origin.
  • Attend discussion and support groups on a quarterly basis to ensure accurate client demographic information.
  • Facilitate support groups and Project TRANS Advisory Committee.
  • Responsible for submitting monthly program impact stories.

Outreach and Event Planning:

  • Serve as an ambassador of Project TRANS by actively engaging in outreach and tabling by having a visible presence at LGBTQ events hosted by The Center and partners.
  • Participate in cross-departmental collaborations including providing on-site support to youth at both youth centers.
  • Assist in the planning and execution of Project TRANS events including Transgender Day of Remembrance Transgender Day of Visibility Trans & Enby Summit Trans Pride Village Gender Affirming Clothing Drives etc.

Administration Data Entry and Reporting:

  • Gather and maintain accurate documentation of clients goals and needs into internal data systems.
  • Assist in completing monthly narrative reports for The Centers Quality Assurance team and external funders.
  • Attend discussion and support groups on a quarterly basis to ensure accurate updated demographic information from clients.
  • Responsible for working with the Project TRANS team to submit monthly program impact stories.
  • Assist in administrative duties to support the Project TRANS program including responding to inquiries creating a monthly programmatic calendar purchases for discussion groups and more.

REQUIRED QUALIFICATIONS:

  • Education and/or Experience: 1 year of experience providing case management services to transgender nonbinary and gender expansive communities.
  • Computer/Office Equipment Skills: Must be comfortable working in PC/Windows-based environments. Must have strong proficiency with Microsoft Office.
  • Other Qualifications:
    • Demonstrated ability to manage multiple tasks with shifting priorities.
    • Must demonstrate competence in working with diverse populations.
    • Must demonstrate competence in using strength-based trauma-informed and motivational interviewing techniques.
  • Language Skills: Fluency in English required.

PREFERRED QUALIFICATIONS:

  • Education: Bachelors degree in a social service-related field.
  • Experience: 2 or more years of experience providing case management services to transgender nonbinary and gender expansive communities. 1 year or more of experience providing case management services to people living with mental illness and/or substance use disorders.
  • Other Qualifications: Knowledgeable about current issues related to the LGBTQIA community.
  • Language: Fluency in Spanish.

WORK LOCATION:

  • On-site in the office and with program participants in the field.
  • Office in the Hillcrest Neighborhood in the City of San Diego.

COMPENSATION & BENEFITS:

  • Compensation: The pay range for this position is $23.00 - $25.00 per hour and is commensurate with experience. This is a Full-Time Regular Non-Exempt position.
