SUMMARY
The Service Coordination Supervisor supports the CHS mission by overseeing the daily operations of the Service Coordination (SC) Department and follows the Department of Mental Health statutes and directives along with CHS policies and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Exercises independent judgmentby planning and coordinating with the Service Coordinator regarding specific issues
- Affirms the Success of Team Membersassigned by recruiting interviewing hiring scheduling and providing necessary training. Provide constructive and timely feedback by scheduled performance evaluations or during everyday interactions to assigned team members. Handles discipline and termination of assigned team members in accordance with company policy by implementing measures focused on motivation of the Service Coordination team to improve performance quality and efficiencies.
- Create a Culture of Communicationby attending and conducting staff team meetings at least once per month by sharing departmental information process changes identifying solutions to issues or concerns raised by team members and working with team members to problem solve case specific issues. Maintaining effective communication and professional relationships with families guardians community providers and other stakeholders.
- Deliver Quality Services to Individualsby ensuring assigned team members are completing documentation accurately and within required timelines to include by not limited to support monitoring TCM logging/billing ISP development quarterly reviews and Utilization Review requests. Serves as a resource to team members by using best practices in working with individuals served and community providers.
- Works closely with the Regional Office PR/QE teamsby monitoring the quality of providers.Staff will comply and understand the unique combination of cultural variablesby appropriately responding to the ongoing emphasis on Cultural Competencies of the organization.

QUALIFICATIONS
REQUIRED EDUCATION:
- Bachelors degree (BA or BS) in related field from an accredited university plus five years of related work experience or masters degree (MA or MS) in related field from an accredited university plus three years of related work experience.
REQUIRED MISCELLANEOUS QUALIFICATIONS:
- Must provide valid drivers license.
- Must provide proof of personal valid vehicle insurance.
PREFERRED EXPERIENCE
- Previous supervisory experience is preferredORtwo years experience as a Lead Service Coordinator at CHS; strong leadership qualities personnel management and exceptional communication interpersonal and organizational skills preferred.
- Experience with a variety of case management and documentation applications such as but not limited to Setworks Kronos Therap etc.
- Experience as a member of a multi-disciplinary team in the areas assessing developing or implementing individual person-centered plans especially for persons with intellectual/developmental disabilities.
- Experience in utilizing the Microsoft Office Suite.
PHYSICAL REQUIREMENTS

- Some travel for meetings within the region.
- Must be able to lift 10 pounds at times and occasionally lift and/or move up to 25 pounds.
