Job Summary
ESSENTIAL QUALIFICATIONS
Masters in Social Work with at least two years of geriatric experience. Strong organizational counseling interpersonal and publicrelation skills. Intermediate computer skills and knowledge of senior community resources Medicare/Medicaid/insurance processes MDS and medical terminology. Able to communicate clearly with all residents including those with vision or hearing challenges. Completion of required assessments orientation annual competencies licenses HIPAA and compliance training.

ESSENTIAL DUTIES
Provide compassionate psychosocial support to residents families and staff. Partner with Nursing Psychology Hospice and other services to meet emotional and medical needs. Assist with admissions transitions referrals crisis situations and care planning. Attend meetings family conferences and support satisfaction surveys. Maintain departmental efficiency and provide staff education. Offer coverage for the RN Case Manager to ensure consistent care.
PERSONAL CARE DUTIES
Support new and respite residents as they adjust offering guidance and reassurance. Address resident and family concerns promptly and collaboratively. Participate in care conferences and assist Admissions by warmly welcoming potential residents and families.
