Licensed Practical Nurse (LPN) Primary Care
Location: Columbia PA 17512
Assignment Duration: 8 Weeks (with possibility of extension)
Schedule: MondayFriday 7:30 AM 4:00 PM
Position Overview
A healthcare organization in Columbia Pennsylvania is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to support a patient-centered primary care setting focused on population health preventive care and meaningful behavior change.
This role supports both clinical and administrative workflows and requires a strong commitment to high-quality patient care and collaboration within a multidisciplinary team.
Key Responsibilities
Provide patient-centered care by measuring and documenting vital signs
Escort and room patients; prepare patients for exams and procedures
Assist providers during examinations and clinical procedures
Document visits accurately in the Electronic Health Record (EHR)
Administer medications and vaccines per clinical guidelines
Perform phlebotomy and collect blood specimens
Triage and process patient phone calls following established protocols
Educate patients on care plans treatments and follow-up instructions
Maintain accurate and up-to-date patient medical records
Monitor and restock medical supplies and exam rooms
Support scheduling patient check-in and clerical functions as needed
Qualifications & Skills
Active Licensed Practical Nurse (LPN) license
Strong communication and interpersonal skills
Proficiency with Electronic Health Record (EHR) systems
Excellent attention to detail and organizational abilities
Sound clinical judgment and critical thinking skills
Ability to work independently with minimal supervision
Commitment to patient education preventive care and community outreach
