What Youll Do
- Evaluate diagnose and treat a broad range of adult medical conditions including chronic illnesses such as diabetes hypertension COPD and heart disease.
- Conduct routine and preventive care visits screenings and wellness checks to promote long-term health and early detection of disease.
- Order interpret and follow up on diagnostic tests and lab work.
- Develop evidence-based personalized care plans in collaboration with patients and their families.
- Manage a continuity panel of adult patients ensuring appropriate follow-up and long-term care.
- Document patient encounters diagnoses and treatment plans accurately and promptly in the electronic health record (EHR) system.
- Coordinate care with specialists behavioral health professionals and other providers as needed.
- Participate in quality improvement efforts and initiatives tied to clinical performance metrics and value-based care goals.
- Deliver culturally sensitive and linguistically appropriate care tailored to Houstons diverse community.
Qualifications
- Completion of an accredited Internal Medicine residency program
- Board Certified or Board Eligible in Internal Medicine
- Unrestricted and active Texas Medical License (M.D. or D.O.)
- Current DEA registration (Texas)
- Current BLS and ACLS certifications
- Proficiency in Spanish or another second language is a strong plus
- Experience in a value-based care or community clinic setting is preferred
- Excellent clinical judgment communication and teamwork skills
- Passion for delivering high-quality patient-centered care to underserved populations

- The national target base salary range for this role is: $200000 - $245000. This salary range represents our national target range for this role. Actual compensation will be determined based on geographic location (current or future) experience and other job-related factors.
- Sign-on bonus and annual performance bonus structure aligned with value-based care outcomes including quality metrics patient engagement and panel management performance
- Astrana employees who refer additional physicians to our practices will be eligible to receive a $3000 bonus per physician hired OR $3500 bonus per bilingual physician hired!
