Job Title: Licensed Clinical Social Worker (LCSW) Travel Contract
Duration: 13 16 Week Contract
Schedule: Day Shift 5x8s (7:00 AM 3:00 PM)
Job Overview
We are seeking a dedicated Licensed Clinical Social Worker (LCSW) for a short-term acute care hospital assignment. This role involves providing comprehensive psychosocial assessments care coordination discharge planning and counseling services in a collaborative healthcare environment.
The ideal candidate is experienced in hospital-based social work proficient in EPIC charting and comfortable working with diverse patient populations.
Key Responsibilities
Assessment & Care Planning
- Conduct comprehensive biopsychosocial assessments
- Develop and participate in individualized treatment plans
- Participate in interdisciplinary care conferences
- Document all patient interactions and care coordination activities in EPIC
Care Coordination & Discharge Planning
- Coordinate services for home care equipment rehabilitation and long-term care facilities
- Assist patients with navigating community resources and public assistance programs
- Refer patients to appropriate internal and external disciplines/resources
- Support collaboration with protective services when appropriate
Counseling & Support
- Provide individual marriage and parental counseling
- Support patients experiencing psychiatric crises including involuntary holds
- Assist patients and families with guardianship power of attorney and protective service needs
Community Resource Knowledge
- Medicaid and Medicare
- TANF and other income-based programs
- Food assistance programs
- Medication assistance programs
- Adult and child protective services
- Adoption and foster care resources
- Substance use treatment options
Requirements
- Active LCSW license (Oregon license required; may submit pending)
- Minimum 1 year of LCSW experience
- Recent acute care or hospital experience preferred
- EPIC charting experience required
- BLS certification required
- Ability to work weekends (every other weekend)
- Must be willing to float to nearby affiliated facilities if needed
- Must have reliable transportation
