The UW Medical Center-Montlake Social Work Departmenthas an outstanding opportunity for a Social Worker - Cystic Fibrosis Clinic.

WORK SCHEDULE

  • Part-Time / 32 hours per week

  • Day Shifts (Monday/Thursday on-site and 2 day telework)

  • This position is expected to last through 6/30/2026 to be renewed annually upon notice of funding

POSITION HIGHLIGHTS

  • The Cystic Fibrosis (CF) Social Worker providesexpertisein helping patients and families with their emotional and practical needs and supports patients andsupport systemsin coping with CF in the differentimpactsoftheir chroniccondition treatmentandinvolved in the transition stages including transition to adult careand across lifespan

  • Be responsible forprovidinga full range of social work services including psychosocial assessment treatment planning therapeutic interventions crisisinterventionand resource referral(s)

  • Partner with the patient and the health care team to coordinate the patients care through effectiveutilizationof resources

PRIMARY JOB RESPONSIBILITIES

  • Complete annualCF Social Workscreening for health-related social needsandsocial drivers of health (SDOH)such ashousing instability/ homelessness transportation food insecurity unreliable access to medicationsandinsurance.

  • Provide specialtyconsultationsto other health care staff and health care teams regarding psychosocially focused aspects of patient carethat contribute to barrierstocare.

  • Providepsychosocial supportandassistancewith care navigationto patientswith complicated social and medical historiesspecifically arounddisability.

  • In collaboration with the multidisciplinary team develop and implement safe andtimelyinterventions to address access to careand services.

  • Coordinationof carealso includes consideration of otheraspectsof comorbiditiesincludingmental and behavioral healthconditions chronicpain substance use sleep executive functioning challenges body image concerns disordered eating and end of lifeconcerns.

  • Alignment and continuous education with CF patient associations attend nationalconferenceandremain current on the evolving disability needs of individuals with CF.

REQUIRED POSITION QUALIFICATIONS

A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENT

Within 90 days of hire must hold one of the following credentials issued by the state of Washington:

Agency Affiliated Counselor

Licensed Social Work Associate -Advanced

Licensed Social Work Associate - Independent Clinical

Licensed Advance Social Worker

Licensed Independent Clinical Social Worker

Compensation Benefits and Position Details

Pay Range Minimum:

$42.61 hourly

Pay Range Maximum:

$66.69 hourly

Other Compensation:

$5000 Sign-On Bonus Certification Premium - $1.50/hour
