An opportunity is now available to join the incredible Cancer Care team for a Nurse Practitioner. This role is based at Nepean Hospital and will also service the Hawkesbury district area.
The Supportive and Palliative Care Nurse Practitioner/Transitional Nurse Practitioner is educated and authorised to function autonomously and collaboratively in an advanced and extended clinical role.
The role provides independent and collaborative expert Palliative Care Nursing for patients with life limiting illness which includes promotion and maintenance of optimum quality of life as well as the provision of individualised supportive and palliative care to the patient and their family/carers.
This position will focus on developing and leading new models of care and collaborative partnerships between the Nepean Blue Mountains Local Health District and key stakeholders to provide better health care and access to specialist palliative care for older people with complex needs who have a life limiting illness. Working as part of a multidisciplinary team you will provide Clinical care timely expert intervention and management of complex care patients referred to the Supportive and Palliative Care Service.
